Adam Perry @ OpenAI a45b8ccb01 Add streaming file replacement to filesystem abstractions (#54505)
## What changed

Add `write_file_stream` to `ExecutorFileSystem` and `EnvironmentAccess`, preserving the accessor's captured sandbox permissions. Local and streaming-capable remote filesystems replace files one bounded chunk at a time and return the byte count. Older executors buffer the input before sending a single `writeFile` request.

Close remote handles on completion or failure, perform best-effort cleanup after cancellation, and invalidate pending metadata lookups. Document that failure or cancellation can leave a truncated or partial file and that in-flight I/O may outlive cancellation.

## Testing

Add coverage for sequential writes, binary and empty files across local and remote filesystems, source failures, handle cleanup, metadata invalidation after cancellation, and compatibility with older executors.

Adam Perry @ OpenAI c39bfa4c8f Enable writable file streaming in exec-server (#50177)
## What changed

Advertise `fileWriteStreaming` and support `fs/open` with `mode: "replace"` to create or truncate a file for writing. Implement `fs/writeBlock` with explicit offsets and nonempty chunks of up to 1 MiB, rejecting ranges beyond the signed 64-bit file offset limit.

Apply write sandbox permissions when opening writable handles, including when full-disk reads are allowed. Keep `read` as the default open mode.

## Testing

Add integration coverage for positional writes, chunk and offset bounds, replacement truncation, read/write handle permissions, and sandbox write restrictions. Verify that duplicate handle IDs and exhausted handle capacity reject replacement opens without creating or truncating files.

Adam Perry @ OpenAI d25c114d49 Bound in-flight file opens in exec-server (#50162)
## Why

The per-connection limit of 128 file read handles was checked only after opening the file, leaving pending opens outside the limit.

## What changed

Reserve a semaphore permit before polling the file-open future, counting pending opens and registered handles against the same limit. Release capacity when an open fails or is cancelled, or when a handle closes, even if a read still retains the file.

Use synchronous handle-table locks that never cross an await, and recheck duplicate IDs after opening to preserve the winning handle during concurrent opens.

## Testing

Add tests for stalled-open progress, pending-open admission limits, permit cleanup after failure or cancellation, capacity reuse while a file remains in use, and concurrent duplicate IDs.

Charlie Marsh f2b2e5b2a9 Make exec-server file reads cancellable between chunks (#49696)
## What changed

Run direct `read_file` operations in a blocking task with bounded read chunks and cancellation checks. Dropping the async caller cancels further reads between chunks. Preserve the 512 MiB size limit, symlink-following options, and regular-file validation.

## Testing

Add regression coverage for 4 MiB reads and oversized-file rejection with both symlink-following options, cancellation after the first chunk, and FIFO rejection without waiting for a writer.

open-matt b8d5e3f12e Allow exec-server to proxy permitted private IPs upstream (#48568)
## Why

Private IP destinations always bypassed inherited upstream proxies, preventing their use for private networks reachable through an upstream VPN proxy.

## What changed

- Add `codex exec-server --proxy-private-ips-via-upstream`, also configurable with `CODEX_EXEC_SERVER_PROXY_PRIVATE_IPS_VIA_UPSTREAM=true`. The setting defaults to disabled.
- Allow permitted RFC 1918, carrier-grade NAT, and IPv6 unique-local destinations to use an applicable upstream proxy. Loopback and link-local destinations retain direct routing, and destination access policy still applies.
- Keep connections direct when no valid upstream proxy applies or `allow_upstream_proxy=false`. Errors after selecting an upstream proxy do not trigger a direct retry.
- Rename `ExecServerRuntimePaths` to `ExecServerRuntimeOptions` and carry the routing setting from executor startup into the managed network proxy.

## Testing

Add routing coverage for private address ranges, special-use addresses, and public targets with the option enabled and disabled. Verify that HTTP and CONNECT requests still enforce destination allowlists and denylists, and update the CLI help snapshot.

Adam Perry @ OpenAI a4ee536f01 Route filesystem reads and writes by their own sandbox permissions (#46122)
## Why

Filesystem reads previously required a sandbox whenever writes were restricted, even with full-disk read permission. This made permitted reads depend on sandbox availability.

## What changed

- Select sandboxing independently for reads and writes, allowing full-disk reads directly while keeping restricted operations sandboxed.
- Use the executor's path convention when evaluating full-disk access, including `:slash_tmp` denials.
- Apply read-specific checks to capability discovery and skill resource reads, allowing unrestricted reads on executors without sandboxed discovery support.

## Testing

Add regression tests for direct read APIs, restricted writes and reads, executor-specific permission rules, and capability discovery without sandbox support. Update Windows tests to verify sandbox enforcement through writes.

Adam Perry @ OpenAI 841b5490b2 Preserve filesystem sandbox policy context when the cwd disappears (#46112)
## Why

Removing the selected working directory can prevent filesystem sandbox helpers from launching, even when the requested absolute paths remain accessible. Permission rules must stay anchored to the selected directory while those operations continue.

## What changed

- Require a policy `cwd` in `FileSystemSandboxContext` and launch filesystem helpers from the filesystem root while preserving the policy directory and workspace roots.
- Carry explicit `policyContext` in filesystem RPCs, preserving legacy wire fields and resolving omitted directories from older clients at executor ingress.
- Keep permission paths as executor file URIs and validate host compatibility where they are enforced.
- Bind Windows relative denial globs to the policy directory before changing the helper's launch directory, preserving home-relative patterns.

## Testing

Add regression coverage for `apply_patch` after working-directory removal, legacy RPC directory fallbacks, cross-platform permission URI transport, and Windows relative read denials. The patch regression verifies that an allowed file is updated while an explicitly denied file remains unreadable and unchanged.

Charlie Marsh 900b1e4cec Add tracing for project instructions and filesystem sandbox operations (#43913)
## What changed

Add tracing spans for `AGENTS.md` discovery and loading, local file reads and metadata lookups, and filesystem sandbox request preparation and execution. Record the instruction byte limit, whether file operations use the sandbox, and the permission entry count during sandbox preparation, while skipping automatic argument capture.

jif eb15245d82 Batch sandboxed capability root discovery (#40443)
## What changed

- Run capability discovery for roots with the same sandbox context in one sandbox helper invocation.
- Keep roots with different sandbox permissions isolated, and fall back to per-root discovery if the batched invocation fails.

## Testing

- Add coverage for single-launch batching, fallback after a sandbox launch failure, and roots with different permission profiles.

cgst-oai a26d50852a Require filesystem backends to implement directory walks (#39749)
## What changed

- Make `ExecutorFileSystem::walk` a required backend operation instead of
providing a fallback built from directory reads and metadata requests.
- Implement bounded local walks on a blocking task with cancellation, symlink
cycle detection, deterministic ordering, error collection, and response-size
  limits.
- Have remote filesystems use the server's walk operation directly.

## Testing

- Cover local and remote handling of invalid roots and limits, directory
symlinks, non-UTF-8 names, cancellation, sandbox contexts, and response
  budgets.

pakrym-oai e3e5ad2847 Harden unsandboxed patch filesystem access (#39659)
## Why

An `apply_patch` path can be replaced with a symlink after verification, allowing an unsandboxed patch operation to reach a different file than the one that was approved.

## What changed

- Add `follow_symlinks` options to executor filesystem reads, writes, metadata lookups, directory creation, and removal, including the corresponding `followSymlinks` protocol fields.
- Implement no-follow filesystem operations on Unix and Windows that reject links in any path component and restrict file access to regular files.
- Run `apply_patch` with symlink traversal disabled when an otherwise-required sandbox is bypassed, while retaining the existing follow-symlink default for standalone callers.

## Testing

- Cover leaf and ancestor symlinks across patch add, update, delete, and move operations, including a path swap after verification.
- Exercise local and remote no-follow filesystem behavior, concurrent directory creation, special-file rejection, and Windows reparse points.

jif c30a3e49c9 Support sandboxed file streaming in exec-server (#38356)
## Why

Streaming reads previously rejected requests that used a platform filesystem
sandbox.

## What changed

- Open streamed files in the sandbox helper and return the open file to the
exec-server by passing a file descriptor on Unix or duplicating a file handle
  on Windows.
- Advertise support through the `sandboxedFileStreaming` environment capability.
- Preserve close-on-exec behavior for transferred descriptors, including the
  required inherited-descriptor cleanup on macOS.

## Testing

- Cover bounded sandboxed streams, continued reads after path replacement, and
rejection of symlink escapes outside readable roots.

Charlie Marsh 40e5de94e9 Avoid redundant filesystem metadata probes (#36898)
## What changed

- Reuse directory-entry file types in local memory listing while continuing to
exclude symlinks, and reuse rollout metadata when reading modification times.
- Avoid following non-symlinks twice in direct filesystem metadata and directory
listing operations while preserving target classification for valid symlinks.

## Testing

- Cover symlink handling in local memory listing and search.
- Extend Unix filesystem tests for followed file and directory symlinks and
  dangling metadata links.

jif c14623d04c Add a bounded filesystem walk RPC (#29841)
Stack 1 of 3. Follow-ups: #29842 and #29844.

## What changes

Adds a general bounded `fs/walk` operation to the exec server.

The operation returns file and directory entries plus recoverable
per-path errors. It skips symlinks, preserves the existing filesystem
sandbox routing, and enforces depth, directory, entry, and response-size
limits.

This PR only defines and wires the filesystem operation. It does not
change any callers yet.

pakrym-oai a4711b88dd [codex] exec-server: stream files in chunks (#28354)
## Why

`fs/readFile` buffers the entire file in one response, which makes large
remote reads expensive and prevents callers from applying backpressure.
We need an opt-in streaming path with bounded block sizes while
preserving the existing single-call API for small and sandboxed reads.

## What changed

- Add `ExecServerClient::stream`, returning a named `FileReadStream`
that implements `futures::Stream` and yields immutable 1 MiB byte
blocks.
- Add internal `fs/open`, `fs/readBlock`, and `fs/close` RPCs.
`fs/readBlock` accepts an explicit offset and length.
- Keep unsandboxed files open between block reads, cap open handles per
connection, and clean them up on EOF, error, stream drop, explicit
close, or connection shutdown.
- Reject platform-sandboxed streaming opens instead of turning the
one-shot sandbox helper into a persistent server. Existing `fs/readFile`
behavior is unchanged.

## Testing

- `just test -p codex-exec-server`
- Integration coverage for 1 MiB chunking, exact block-boundary EOF,
sandbox rejection, and continued reads from the opened file after path
replacement.
- Handle-manager coverage for non-sequential offsets, variable block
lengths, the 128-handle limit, and capacity release after close.

Adam Perry @ OpenAI 968a3ac9c1 [codex] make PathUri::from_abs_path infallible (#27976)
## Why

`PathUri::from_abs_path` can fail for absolute paths that do not have a
normal `file:` URI representation, forcing filesystem call sites to
handle a conversion error even though the original path can be preserved
losslessly.

## What

Make `from_abs_path` infallible and migrate its callers. Unrepresentable
paths use `file:///%00/bad/path/<base64>`, encoding Unix bytes or
Windows UTF-16LE; `to_abs_path` validates and decodes that fallback. The
leading encoded null reserves a namespace that cannot collide with a
real Unix or Windows path, and fallback URIs remain opaque to lexical
path operations.

## Validation

Added path-URI coverage for Unix null and non-UTF-8 paths, Windows
device/verbatim and non-Unicode paths, serialization, malformed
fallbacks, opaque lexical operations, invalid native payloads, and
literal `/bad/path` collision resistance.

pakrym-oai 76d8f20241 [codex] Add size to internal filesystem metadata (#27927)
## Why

`ExecutorFileSystem::get_metadata` reports file kind and timestamps but
not size. Internal callers that need to enforce a size limit therefore
have to read the complete file first, which is especially wasteful for
remote filesystems.

This adds the missing internal metadata so consumers can reject
oversized files before transferring or buffering them. The field is
named `size`, matching VS Code's `FileStat.size` filesystem convention.

## What changed

- add `size: u64` to internal `FileMetadata`
- populate it from the underlying filesystem metadata
- carry it through sandbox-helper and remote exec-server responses
- cover files, directories, symlink targets, and sandboxed reads across
local and remote filesystem implementations

The new field is intentionally not exposed through the app-server API.

Adam Perry @ OpenAI 5a56caf18c [codex] Remove async_trait from first-party code (#27475)
## Why

First-party async traits should expose their `Send` contracts explicitly
without requiring `async_trait`. This completes the migration pattern
established in #27303 and #27304.

## What changed

- Replaced the remaining first-party `async_trait` traits with native
return-position `impl Future + Send` where statically dispatched and
explicit boxed `Send` futures where object safety is required.
- Kept implementations behavior-preserving, outlining existing async
bodies into inherent methods where that keeps the diff reviewable.
- Removed all direct first-party `async-trait` dependencies and the
workspace dependency declaration.
- Added a cargo-deny policy that permits `async-trait` only through the
remaining transitive wrapper crates.
- Updated `rand` from 0.8.5 to 0.8.6 to resolve RUSTSEC-2026-0097 and
keep the full cargo-deny check passing.

## Validation
