## What changed
- Reformat Markdown tables and lists, login pages, inline visualization HTML, and sample JavaScript scripts.
- Escape union-type pipes in Python SDK API tables so they render within their cells.
- Keep the formatting-recipe test focused on the working directory and formatter commands by removing assertions on comment text.

## Why
Some metrics backends require cumulative metrics, while configured OTLP exporters previously always used delta temporality.
## What changed
Read `OTEL_EXPORTER_OTLP_METRICS_TEMPORALITY_PREFERENCE` for configured HTTP and gRPC metrics exporters. Accept `delta`, `cumulative`, and `lowmemory` case-insensitively, retaining delta for unset, empty, or unrecognized values. Document the setting and preserve existing temporality for the built-in Statsig exporter and runtime snapshots.
## Testing
Update the HTTP loopback test to set the preference in an isolated subprocess and assert that the exported counter uses cumulative aggregation temporality.

## What changed
Emit `codex.skill_invocation` logs for explicit skill injection and detected implicit use, including skill-tool reads. Include skill name, invocation type, conversation, turn, model, and client metadata, plus scope, plugin ID, and ChatGPT user and account IDs when available.
Exclude skill contents, resource paths, email addresses, prompts, and tool payloads. Use the existing OTEL log exporter without an additional opt-in; `otel.exporter = "none"` continues to export no logs.
Document the event fields, exporter configuration, and detection semantics in `codex-rs/otel/README.md`.

## What changed
Add `otel.log_guardian_assessments`, disabled by default and requiring an OTLP HTTP or gRPC log exporter. Emit `codex.guardian_assessment` when a synchronous Guardian review finishes, including review identifiers, status, risk level, user authorization, and rationale. Include `allow` or `deny` outcomes only for completed model assessments, not review errors, timeouts, or cancellations.
Cap exported rationales at 64 KiB on a UTF-8 character boundary and record their original byte length and truncation status. Use the log-only route to exclude assessment payloads from trace export, local state logs, and bug-report log buffers. Cached V2 approvals are not included.
## Testing
Add coverage for configuration gating, enabled and disabled denial logging, terminal statuses and outcomes, UTF-8 rationale truncation, export routing, and bug-report log buffer exclusion.

## What changed
Add `otel.log_agent_responses`, disabled by default and requiring an OTLP HTTP or gRPC log exporter. Emit `codex.agent_response` for completed `final_answer` messages from main agents and spawned task agents, with session, turn, item, and available parent/root attribution.
Strip memory citations from exported text, preserve the stored answer, and cap responses at 64 KiB on a UTF-8 character boundary. Include the original byte length and a truncation flag, and suppress duplicate item completions within each turn. Exclude commentary and non-task agents.
## Testing
Extend configuration tests for default-off and explicit opt-in behavior, the agent execution test for main/subagent response attribution, and the log-buffer test for response-event exclusion.

## Why
Per-socket metrics label resends after reconnect as initial requests, obscuring why full input was sent. Client telemetry can preserve that reason across reconnect failures and turn boundaries.
## What changed
- Add `codex.websocket.continuation` to count `response.create` send attempts, including failed sends, with `mode`, `reason`, and `phase` tags.
- Distinguish incremental continuations, fresh requests, restored history, closed connections, and other full-input sends. Track warmup and generation separately.
- Preserve the first continuation reset reason until the next send and identify resumed or forked history.
- Allow session telemetry to use a `MetricsClient` supplied through thread extensions and document the counter's semantics.
## Testing
Add metric assertions for incremental reuse, warmup, changed requests, account switches, reconnects, and resumed or forked history. Add a unit test for preserving reset reasons across repeated resets and turn boundaries.

## Why
Creating the telemetry shutdown thread during shutdown can fail under resource
pressure, including when the native thread guard page cannot be allocated.
## What changed
- Start and verify a dedicated shutdown worker when `OtelProvider` is created.
- Send the provider to the prepared worker for bounded shutdown, while preserving
timeout behavior and avoiding a potentially blocking destructor if worker
preparation failed.
- Rename the fallible provider constructor to `try_new`.
## Testing
Add Unix regression coverage that injects guard-page allocation failures, plus
coverage for worker preparation failure, successful shutdown, and timeouts.

Add Codex config for static trace span attributes and structured W3C
tracestate field upserts. The config flows through OtelSettings so
callers can attach trace metadata without touching every span call site.
Apply span attributes with an SDK span processor so every exported
trace span carries the configured metadata. Model tracestate as nested
member fields so configured keys can be upserted while unrelated
propagated state in the same member is preserved.
Validate configured tracestate before installing provider-global state,
including header-unsafe values the SDK does not reject by itself. This
keeps Codex from propagating malformed trace context from config.
Update the config schema, public docs, and OTLP loopback coverage for
config parsing, span export, propagation, and invalid-header rejection.
## Summary
This PR fixes OTLP HTTP trace export in runtimes where the previous
exporter setup was unreliable, especially around app-server usage. It
also removes the old `codex_otel::otel_provider` compatibility shim and
switches remaining call sites over to the crate-root
`codex_otel::OtelProvider` export.
## What changed
- Use a runtime-safe OTLP HTTP trace exporter path for Tokio runtimes.
- Add an async HTTP client path for trace export when we are already
inside a multi-thread Tokio runtime.
- Make provider shutdown flush traces before tearing down the tracer
provider.
- Add loopback coverage that verifies traces are actually sent to
`/v1/traces`:
- outside Tokio
- inside a multi-thread Tokio runtime
- inside a current-thread Tokio runtime
- Remove the `codex_otel::otel_provider` shim and update remaining
imports.
## Why
I hit cases where spans were being created correctly but never made it
to the collector. The issue turned out to be in exporter/runtime
behavior rather than the span plumbing itself. This PR narrows that gap
and gives us regression coverage for the actual export path.
## Summary
This is a purely mechanical refactor of `OtelManager` ->
`SessionTelemetry` to better convey what the struct is doing. No
behavior change.
## Why
`OtelManager` ended up sounding much broader than what this type
actually does. It doesn't manage OTEL globally; it's the session-scoped
telemetry surface for emitting log/trace events and recording metrics
with consistent session metadata (`app_version`, `model`, `slug`,
`originator`, etc.).
`SessionTelemetry` is a more accurate name, and updating the call sites
makes that boundary a lot easier to follow.
## Validation
- `just fmt`
- `cargo test -p codex-otel`
- `cargo test -p codex-core`
## Summary
This is a structural cleanup of `codex-otel` to make the ownership
boundaries a lot clearer.
For example, previously it was quite confusing that `OtelManager` which
emits log + trace event telemetry lived under
`codex-rs/otel/src/traces/`. Also, there were two places that defined
methods on OtelManager via `impl OtelManager` (`lib.rs` and
`otel_manager.rs`).
What changed:
- move the `OtelProvider` implementation into `src/provider.rs`
- move `OtelManager` and session-scoped event emission into
`src/events/otel_manager.rs`
- collapse the shared log/trace event helpers into
`src/events/shared.rs`
- pull target classification into `src/targets.rs`
- move `traceparent_context_from_env()` into `src/trace_context.rs`
- keep `src/otel_provider.rs` as a compatibility shim for existing
imports
- update the `codex-otel` README to reflect the new layout
## Why
`lib.rs` and `otel_provider.rs` were doing too many different jobs at
once: provider setup, export routing, trace-context helpers, and session
event emission all lived together.
This refactor separates those concerns without trying to change the
behavior of the crate. The goal is to make future OTEL work easier to
reason about and easier to review.
## Notes
- no intended behavior change
- `OtelManager` remains the session-scoped event emitter in this PR
- the `otel_provider` shim keeps downstream churn low while the
internals move around
## Validation
- `just fmt`
- `cargo test -p codex-otel`
- `just fix -p codex-otel`
Add metrics capabilities to Codex. The `README.md` is up to date.
