Update the Guardian extra-policy snapshot for retained instructions (#47287)

## What changed

Update `extra_policy_reaches_guardian_review` to expect a retained user
instructions section before the transcript, including the source-order notice
and the original user request. Adjust the subsequent snapshot item numbers.
